Construction began in 1954, with the last sections opening in 1971.

Sometime in 2025, the segment of I-81 between Exits 16A and 29 will be relocated to the current I-481 alignment as the elevated section of I-81 in downtown Syracuse is expected to be demolished and replaced with a grade level boulevard. Construction began in 2023 with reconstruction of the two interchanges with I-481 north and south of Syracuse. The project as a whole is expected to be completed by 2028.

The process to convert I-81's exit numbering scheme from sequential to mileage based began in 2023 with the exits in NYSDOT Region 3 (Cortland, Onondaga, and Oswego Counties) along sections that will not be relocated. Exit numbers in Broome County made the switch in early 2025. Exit numbers north of Syracuse reflect the new mileage once the Syracuse section of I-81 is relocated to I-481.
